Scenario:

On the first nice weekend of April, two young men take their mountain bikes to the Elk Ridge Trails. With taunts as to who is "King of the Mountain," they race off down the trail. One of the cyclists loses control of his bike and crashes into the underbrush. He is launched over the handlebars and lands on the ground, shaken and confused.

His friend quickly rides back to the ranger station to call 9-1-1.

You and your partner arrive to find a 23-year-old man still down on the trail. He is wearing shorts, sandals, and sunglasses but no helmet. The patient is confused and disoriented but responds to pain by withdrawing when you move his arms or legs. Vital signs include a pulse of 60 beats/min, respirations of 28 breaths/min and irregular, and a blood pressure of 166/98 mm Hg. Additional examination shows multiple abrasions over his entire body and an angulated fracture of his left tibia.
